The Heights Baptist Event Request Form

For all Ministry Events: Bulletin, Calendar, Room Requests, etc. Requests must be filled out and returned at least three weeks prior to the event. Requests will not be taken by phone. Do not commit to a date or contract until the request is approved! If the event involves promotion, allow eight weeks for events requiring registration and four weeks for those that do not in order to maximize the opportunity to reach the intended audience.
